RESUMO

The treatment of hydrocephalus often involves the placement of a shunt catheter into the cerebrospinal ventricular space, though such ventricular catheters often fail by tissue obstruction. While diverse cell types contribute to the obstruction, astrocytes are believed to contribute to late catheter failure that can occur months after shunt insertion. Using in vitro microfluidic cultures of astrocytes, we show that applied fluid shear stress leads to a decrease of cell confluency and the loss of their typical stellate cell morphology. Furthermore, we show that astrocytes exposed to moderate shear stress for an extended period of time are detached more easily upon suddenly imposed high fluid shear stress. In light of these findings and examining the range of values of wall shear stress in a typical ventricular catheter through computational fluid dynamics (CFD) simulation, we find that the typical geometry of ventricular catheters has low wall shear stress zones that can favour the growth and adhesion of astrocytes, thus promoting obstruction. Using high-precision direct flow visualization and CFD simulations, we discover that the catheter flow can be formulated as a network of Poiseuille flows. Based on this observation, we leverage a Poiseuille network model to optimize ventricular catheter design such that the distribution of wall shear stress is above a critical threshold to minimize astrocyte adhesion and growth. Using this approach, we also suggest a novel design principle that not only optimizes the wall shear stress distribution but also eliminates a stagnation zone with low wall shear stress, which is common to current ventricular catheters.

OBJECTIVES. To clarify the use of ultrasonography by determining the frequency of developmental dysplasia of the hip among breech-presented Chinese neonates in Hong Kong. DESIGN. Prospective case series. SETTING. Regional hospital, Hong Kong. PATIENTS. All breech-presented Chinese neonates born during January 2008 to June 2009 were included (except premature neonates). They were examined clinically from birth till the age of 1 year. Ultrasound of the hips was performed at the age of 2 weeks, and X-ray of the pelvis at the age of 1 year. RESULTS. A total of 209 breech-presented neonates were born during the study period; 110 neonates completed all necessary investigations and follow-up. Among the latter, there were three neonates with developmental dysplasia of the hip warranting treatment, which amounted to a frequency of 2.7%. CONCLUSION. Developmental dysplasia of the hip among breech-presented Chinese babies is only slightly less common than in corresponding populations in other regions in the world. Since early diagnosis is important, ultrasonography screening in high-risk cases such as those with breech presentation may be useful.

UNLABELLED: This paper presents formaldehyde and volatile organic compounds (VOC) concentrations, potential sources and impact factors in 100 homes. The 24-h average formaldehyde concentration in 37 homes exceeded the good class of the Hong Kong Indoor Air Quality Objectives (HKIAQO), whereas the total VOCs concentration in all homes was lower than the HKIAQO. Compared to other East Asian cities, indoor formaldehyde and styrene in Hong Kong was the highest, reflecting that the homes in Hong Kong were more affected by household products and materials. The formaldehyde concentration in newly built apartments was significantly higher than that in old buildings, whereas no relationship between the concentration and the building age was found for VOCs. There was no difference for formaldehyde and toluene between smoking and non-smoking homes, suggesting that cigarette smoking was not the major source of these two species. Homes of a couple with a child had higher formaldehyde and acetic acid concentrations, while homes with more than three people had higher concentrations of 1-butanol, heptane and d-limonene. When shoes were inside the homes, heptane, acetic acid, nonane and styrene concentrations were statistically higher than that when shoes were out of the homes. Furthermore, higher levels of 1,2,4-trimethylbenzene, styrene, nonane and heptane were found in gas-use families rather than in electricity-use homes. PRACTICAL IMPLICATIONS: Long-term exposure to formaldehyde and volatile organic compounds (VOC) in indoor environments may cause a number of adverse health effects such as asthma, dizziness, respiratory and lung diseases, and even cancers. Therefore, it is critical to minimize indoor air pollution caused by formaldehyde and VOCs. The findings obtained in this study would significantly enhance our understanding on the levels, emission sources and factors which affect indoor concentrations of formaldehyde and VOCs. The results can help housing designers, builders, home residents, and housing department of the government to improve indoor air quality (IAQ) by means of appropriate building materials, clean household products and proper life styles. It can also help policy makers reconcile the IAQ objectives and guidelines.

A non-symmetrical Bouc-Wen model is proposed in this paper for magnetorheological (MR) fluid dampers. The model considers the effect of non-symmetrical hysteresis which has not been taken into account in the original Bouc-Wen model. The model parameters are identified with a Genetic Algorithm (GA) using its flexibility in identification of complex dynamics. The computational efficiency of the proposed GA is improved with the absorption of the selection stage into the crossover and mutation operations. Crossover and mutation are also made adaptive to the fitness values such that their probabilities need not be user-specified. Instead of using a sufficiently number of generations or a pre-determined fitness value, the algorithm termination criterion is formulated on the basis of a statistical hypothesis test, thus enhancing the performance of the parameter identification. Experimental test data of the damper displacement and force are used to verify the proposed approach with satisfactory parameter identification results.

Astragalus membranaceus, a commonly used Chinese medicinal plant, has been shown to be capable of restoring the impaired T cell functions in cancer patients. In this study, the in vitro and in vivo anti-tumor effects of A. membranaceus were investigated. Five bioactive fractions were isolated from the root of A. membranaceus, the fraction designated as AI was found to be the most potent among the five fractions with respect to its mitogenicity on murine splenocytes. Besides investigating the cytostatic effect of AI, its activities on macrophage function, tumor necrosis factor production, induction of lymphokine-activated killer cell and tumor cell differentiation were also examined. The macrophage-like tumors and the myeloid tumors were found to be more sensitive to the cytostatic activity of AI, whereas the fibroblast-like tumors and the mouse Ehrlich ascites tumor appeared to be relatively resistant. Moreover, AI could effectively suppress the in vivo growth of syngeneic tumor in mice. Results showed that murine macrophage pretreated with AI had increased in vitro and in vivo cytostatic activities towards MBL-2 tumor. AI could also act as a priming agent for tumor necrosis factor production in tumor-bearing mice. Preincubation of mouse splenocytes with AI could induce in vitro lymphokine-activated killer-like activity towards WEHI-164 cell. Furthermore, AI was able to induce monocytic differentiation of both human and murine cells in vitro. AI administered in vivo could even partially restore the depressed mitogenic response in tumor-bearing mice. Collectively, the results showed that A. membranaceus could exhibit both in vitro and in vivo anti-tumor effects, which might be achieved through activating the anti-tumor immune mechanism of the host.
